News about Box 32; Liz Jones, Medical Association of Billers

Are you having difficulty getting some of your claims paid? Did you know that on all claims after April 1, 2005, The Centers for Medicare and Medicaid Services require that Box 32 of the CMS-1500 form be completed for each practice location, except for those services performed in the Beneficiary's home? Post office boxes are not acceptable, nor will writing "Doctor's Office" in Box 32 be acceptable. Claims prior to April 1, 2005 can still have Box 32 blank if the information in Box 32 is the same as Box 33.

On that same note, claims received on/after April 1, 2004 for payment of services by Medicare, will be based on the zip code of where the service was provided. What does this mean? Again, Box 32 must have the name, address and zip code of the service location.
